Are you a dedicated and compassionate Social Worker looking to make a meaningful difference in the lives of adults within our community? We are seeking an experienced Social Worker to join our supportive and forward thinking Adult Social Care team.
About the Role
In this full time position, you will play a key role in delivering high quality, person centred social work practice. You will work with adults with a range of complex needs, empowering individuals to live independently and safely while ensuring statutory duties and best practice standards are met.
Key Responsibilities
- Undertake strengths based assessments and develop personalised care and support plans
- Work collaboratively with health partners, voluntary services, carers, and families
- Manage a varied and complex caseload with confidence and professionalism
- Apply legislation including the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures
- Make defensible decisions that promote wellbeing and positive outcomes
- Maintain accurate and timely records in line with organisational and professional standards
About You
We are looking for someone who is:
- A registered Social Worker with relevant post qualification experience
- Skilled in assessment, risk management, and safeguarding adults
- Confident in working autonomously while being a strong team player
- Able to build effective relationships with service users, families, and partner agencies
- Passionate about delivering excellent social work practice and continuous improvement
