A rewarding opportunity has arisen for a Social Worker to join our dedicated Community Health and Social Care team in the Ivybridge and Kingsbridge area.
The role focuses on supporting individuals who have experienced a stroke, along with their families and carers. This is a 12 month secondment opportunity for internal candidates.
You will play a key part in supporting people to regain independence, through a strengths based approach. You will work closely with multidisciplinary colleagues, while ensuring that every person receives personalised, holistic and empowering support.
Key qualities: up to date experience of completing Care Act assessments, Mental Capacity assessments and Safeguarding. You will be a case holder, working both in a hospital and community setting. You will require strong communication skills and the ability to challenge and navigate difficult situations.
