Team Manager for Adults of Working Age:

Disability Living Review Team - Exeter

The Disability Living Review Team is a County-wide team.  It has developed and established a new approach to promoting independence with adults with disabilities. You would be able to demonstrate effective close working in partnership with families and providers. You will be managing a strong, established team who are confident in an approach which promotes positive outcomes. The requirements of the role call for good organisational skills in terms of planning and co-ordinating a programme of reviews.

Team Manager for Adults of Working Age:

Community Health and Social Care Team covering Honiton Ottery St Mary and Sidmouth Axminster and Seaton

Your role in the management team is to bring a focus on the work to support adults of working age with disabilities, ensuring that all parts of the health and care system keep the person at the centre and work to promote their independence.  Your priority will be to provide a particular focus on assessments for young people who are Preparing for Adulthood, build links with our local special education providers and co-ordinate representation at school reviews.  You will be confident working with complexity and challenge and able to chair multi-agency safeguarding and Best Interest meetings.  You will take a lead in identifying and managing the need for assessments in relation to Community Deprivation of Liberty.  You will provide line management support to social care staff, link with community and health providers and will have opportunities to support new initiatives and new approaches to commissioning support.

Team Manager – Adults of Working Age

Community Health & Social Care Team based at Exmouth Hospital

This is an opportunity to join a strong and stable health and care management team serving the communities of Exmouth, Budleigh and Woodbury in East Devon.  You will be managing those social care staff who predominantly work with adults of working age, with opportunities to collaborate with colleagues and managers to work in partnership with the voluntary sector and local communities, implementing innovative approaches. There is a particular focus on embedding integrated working with all colleagues, to include the Urgent Community Response Team, modelling a ‘one team’ approach.

Team Manager - Assessment and Support Planning 

Care Direct Plus Eastern - Exeter

A rare opportunity has arisen to join the public-facing assessment and support planning team with Care Direct Plus in Eastern Devon, where you will be leading a dedicated team of social care assessors who work with older people and adults with a physical and learning disability, offering advice and completing assessment of need primarily via the telephone. Working as part of a management team you will promote strengths-based practice, provide expert advice and oversee quality and workflow. You will also represent the service in terms of partnership working with a range of stakeholders, and have the opportunity to contribute to policy and practice development.

The Team Manager role within Care Direct Plus will be working a 5 day week on a staff rotation basis, over six days; Monday to Saturday, office hours.

Saturday working attracts an enhanced rate. Currently staff are required to work on average one Saturday every 9 weeks, though this may be subject to change to meet the needs of the service.

Team Manager – Focus on Older Adults

Barnstaple Health & Social Care Team – 0.8 Post

This is an exciting opportunity to join Barnstaple Community Health and Social Care Team for a 2 year fixed-term secondment.

We are looking for a dynamic manager, who can demonstrate commitment to the principles of quality and continuous improvement, in relation to team and service user outcomes.

The successful applicant will join an established social care team, which is part of a larger, co-located and integrated community service, working together to support the residents of Barnstaple. The main focus of work will be older persons services; however, there will be a need to be flexible in supporting workers with a wider range of casework. The post-holder will also need to guide the team through new challenges as we move through a process of learning, to develop our services, and to promote understanding and wellbeing.

 As a Team Manager you will provide:

•           Excellent leadership to ensure the delivery of Social Care outcomes through the provision of effective line management, comprehensive supervision and casework monitoring.

•           Effective communication skills as you work in partnership with statutory and voluntary sector oganisations. 

•           Commitment to involving service users and carers in the delivery of a high quality and equitable service.

You will need to demonstrate that you possess these core competences to underpin achieving excellent practice and performance.

About You

You will have excellent knowledge of health and social care.  You will be registered with HCPC as a Social Worker or Occupational Therapist, holding the required qualification and having completed the requirements of post-qualifying learning (for example the assessed and supported year in employment or preceptorship). You will evidence your experience gained from your previous professional roles and their relevance to this post. Three years post qualifying experience of Social Work or Occupational Therapy is required, across different roles or working in different teams.
