Care Direct Plus, part of Adult Care & Health, has a permanent, full-time (37 hours per week) and a temporary, fixed term full-time vacancy for a Social Care Assessor based in Barnstaple, covering Northern Devon. This is an exciting opportunity if you are interested in progressing your career in the social care field. You will learn new skills and be supported as you gain experience through regular supervision from members of the team. Devon County Council is investing in its social care workforce to ensure that the people of Devon can access appropriate community assets and resources. Be part of our ongoing commitment to excellent delivery and integrated care.
You will be working in the Social Care Assessment Team at Care Direct Plus North to offer advice and complete assessments for younger adults and older people via the telephone. You will be supporting people to retain their independence and taking a variety of contacts and referrals from the public and other professionals, providing advice and information, undertaking holistic assessments and support planning, working in partnership with other agencies, making onward referrals and reviewing services.
We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers and other professionals. You will have experience of working in a health or social care environment and have a sound understanding of human needs. You will also ideally have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team.
You should be an effective communicator, both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have effective time management skills and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to effective customer service and to achieving positive outcomes for vulnerable people.
In return, you will be part of a friendly, supportive and experienced team with regular supervision, appraisal, regular team meetings and access to learning and development opportunities.
The Social Care Assessor role within Care Direct Plus will be working a 5 day week on a staff rotation basis, over six days; Monday to Saturday, office hours. Saturday working attracts an enhanced rate. Currently, staff are required to work on average one Saturday every 12 weeks, though this may be subject to change to meet the needs of the service.
Current Social Care Assessors should apply using the internal transfer process and the attached expression of interest form. Please speak to your manager in the first instance.
Please note; the interview process will also consist of a 30 minute written exercise.
We are committed to safeguarding and promoting the welfare of children and vulnerable adults, and expect all staff and volunteers to share this commitment.
