Social Care Assessor (Ref.2728)

This post will sit within the East Exeter Social Care Team with duties to assess and review people who may have eligible needs as defined under the Care Act (2014), to commission best value care packages and signpost to universal services where a person is not deemed eligible by the LA. 

The role includes working with partner agencies to ensure people’s needs are met appropriately and efficiently.

There is a requirement to support CHC applications, an expectation for leading on MCA, involvement with Best Interest decisions and support Court of Protection applications.  SCA will complete STS work to assess those discharged from Community hospitals within 4 weeks to ensure  timely and accurate placements or ensure safe return to community adhering to process and procedures.
