This is an exciting opportunity to work within our new inpatient service which is being provided by Cygnet Healthcare in Taunton.

Sycamore Ward is a 16 bedded all male ward based in Taunton.

The role of the Discharge Facilitator will be to work with the inpatient service, the Crisis Resolution Home Treatment Team and the community based teams to support the safe discharge of the individuals.

The successful applicant will need to have excellent communication skills, both written and verbal and be able to work in a challenging and demanding environment. Experience of working within housing sectors, councils and accommodation roles would be advantageous.

The role will be mainly based in Taunton but will require frequent travel and meetings to Torbay.

This is a fixed term position for two years.

As a new member of staff to Devon Partnership NHS Trust you will be required to attend a full induction programme at the beginning of your employment. The content and length of this programme will depend on your new job role and where you will be working. This can be between one day for non-clinical staff and up to 10 days for inpatient clinical staff.
